• Occasional Wednesdays
• Emergency Care
• A minimum of 24-hour advance cancellation notice requested.
• No-shows will be charged for missed appointments.

• We adhere to all OSHA, FDA and CDC requirements

• We participate in Delta, Blue Cross, United Concordia plans, United Healthcare

• Payment is requested at time of services.

Your first visit? What to expect:
• If possible, please have your current dental records forwarded at least one week prior to your first appointment.
• Be sure to bring with you all insurance information and be prepared to complete a brief medical history, including recent and current medications.
